Forms for the application of approval

FORMS FOR THE APPLICATION OF APPROVAL  OF A PROJECT BY THE ETHICS COMMITTEE

Below are the forms to be completed by the Scientific Research Project Manager for the application of approval of the project by the Ethics Committee of the National Centre for Social Research:

1. Application Form
2. Consent /Information Form
3. Researchers' Confidentiality Statement
4. Additional Confidentiality Agreement for personal data processing/ Additional Terms and Conditions for personal data processing

The Additional Confidentiality Agreement for personal data processing is required only if the Scientific Research Project Manager assigns, on the basis of an agreement, the data processing to an external partner. It does not need to be completed if the members of the project team undertake the processing of the data.
